This is how to fill your form:

1. Fill in the form given here by the right hand side of the screen by providing your Name, Email and Phone number
2. Click on the “create” button to create your account information.
3. Print the login details displayed after clicking “create” button in step(2) above
4. Go back to the school portal at ” https://sis.conssokoto.edu.ng”, enter the username and password printed in the step (3) above and click on “ login” button.
5. Click on the “Applicant” tab at the left hand side of the screen to print an invoice for form fee and portal access fee.
6. Take the invoice to any of the following banks to make payment
1. Firstbank
2. Fidelity bank
3. Heritage bank
4. Polaris bank
5. Stanbic IBTC bank
7. Go back to the school portal at ” https://sis.conssokoto.edu.ng”, enter the username and password printed in the step (3) above and click on “ login” button.
8. Click on the “Applicant” tab at the left hand side of the screen and click on the link “Continue with application” to fill the form.
9. Make sure to print the form for reference purpose.
